Clinical Pilates at Vital Core Physiotherapy: A more personalised approach to exercise rehabilitation

While many people know the term Clinical Pilates, we prefer the name Clinical Rehab because it better reflects what we do. Rather than everyone completing the same sequence of exercises, each person follows a program developed specifically for them by their physiotherapist.

From Clinical Pilates to Modern Clinical Rehabilitation

Joseph Pilates developed his exercise method more than 100 years ago. During World War I, he worked with injured soldiers and began experimenting with springs attached to hospital beds, helping patients rebuild strength and mobility while recovering from injury. These early innovations became the foundation for the reformers, trapeze tables and other equipment still commonly associated with Pilates today.

When Pilates later moved to New York, his methods became particularly popular among dancers and athletes who valued the focus on strength, control, flexibility and efficient movement.

Much has changed since then. Over the past several decades, physiotherapists around the world have adopted many of the principles behind Pilates while incorporating advances in exercise science, pain management and rehabilitation. What began as Pilates-based exercise has evolved into a much broader and more individualised approach to helping people move better, become stronger and remain active throughout life.

At Vital Core, we continue to use many of the pieces of equipment associated with Clinical Pilates, including reformers, trapeze tables and Pilates chairs. However, the equipment is simply a tool. What matters most is selecting the right exercise, at the right time, for the right person.

One of the enduring strengths of Pilates is its focus on how we move, not simply how much we move.

As physiotherapists, we often see people who have stopped trusting their bodies following an injury, surgery or period of persistent pain. They may have become weaker, more cautious, or developed movement habits that no longer serve them well. In these situations, exercise becomes about much more than building fitness. It becomes an opportunity to restore confidence.

This is where Clinical Rehab can be particularly valuable. By carefully selecting and progressing exercises, we help people reconnect with movement, improve balance and coordination, and develop strength in a way that feels achievable and meaningful. What Makes Vital Core's Clinical Rehab Classes Different?

The biggest difference between our Clinical Rehab classes and many traditional Pilates or fitness classes is that every participant follows their own individualised program.

Walk into one of our classes and you'll see something quite different from a typical group exercise session. One person may be working on balance exercises after a fall. Another may be rebuilding strength following knee surgery. Someone else may be focusing on managing osteoporosis, while another participant is returning to exercise after having a baby. Although they are exercising together, each person is working toward their own goals and completing a program specifically designed by their physiotherapist.

Our classes are intentionally small, with typically only four to five participants per session. This allows our physiotherapists to provide ongoing guidance, adjust exercises when needed and ensure everyone continues progressing safely and effectively. The atmosphere is supportive and relaxed, often appealing to people who feel overwhelmed by larger gyms or who simply prefer a more personalised environment.

For many clients, this small-group approach provides the best of both worlds. They enjoy the motivation and social connection that comes from exercising alongside others, while still receiving the individual attention that their rehabilitation requires.

Starting Clinical Rehab at Vital Core Physiotherapy

Before joining a class, you'll spend time working one-on-one with your physiotherapist. During these sessions, we'll assess your movement, strength, balance, goals and any physical limitations before developing a personalised exercise program.

We'll teach you the exercises, introduce you to the equipment and help you build confidence before transitioning into one of our small group Clinical Rehab classes. This process ensures you understand your program and gives us the opportunity to tailor exercises to your specific needs from the very beginning.

As your body changes and your goals evolve, your program evolves too. Exercises can be progressed, modified or refined to ensure you continue moving forward. This is one of the reasons many people remain in Clinical Rehab long after an injury has resolved—they appreciate having an exercise program that grows with them.
